General education benefits
All Plan Participants were mailed an April 2016 memo, General Education Application, Short Course Request and Reimbursement Request. If you would like an electronic copy of any of these forms, please contact our office at (808) 847-0629.
Application For General Education Benefits
For the regular school year (Fall-Spring), applications must be received on or before October 31st of the current school year.
For Summer, applications must be received on or before April 30th of the current summer session.
Short Course - Request For Reimbursement
This application must be submitted together with a certificate of completion of the course and proof of payment no later than 30 days following the completion of the seminar or course. No late applications will be considered.

Please attach the following:
Documented receipt (statement) for tuition and fees per semester.
Any grants/scholarships received that offset tuition and/or fees.
Copy of school issued registration/course schedule per semester.
Copy of grade transcript for each term/semester/session.
Incomplete applications will not be accepted. Deadline to submit reimbursement form and documents: 30 days following the completion of them most recent term/session.
